Goal Setting form

Write down 1-3 goals you want me to help you achieve. Include at least one short term goal (3 mos.) and at least one long term (12 mos.) I want these goals to be at least 3 layers deep, i.e. what actions are you going to take to achieve your goals?

Your goals should be SMART:

Specific

Measurable

Action-Oriented

Realistic 

Time-bound

Example:

  1. I want to lose 20 pounds by April

    a) I’ll do this by strength training at gym 3 days a week

    b) I will make sure I’m productive at the gym by following a plan written by my trainer

    c) I’ll carve out time in my schedule on Monday, Wednesday, and Friday and bring my gym clothes to work so I don’t have to go home first.
